Erectile dysfunction
There are numerous causes for erectile dysfunction. Men over 50 often experience it as symptom of arteriosclerosis or high blood pressure. In these cases the occurrence of erectile dysfunction can be an early signal for other diseases which might require immediate treatment.
The penis erects when the arteries around the cavernous body dilate and allow an increased bloodstream.
For it the vascular muscels have to relax. During sexual stimulation the increase of nitrogen monoxide causes the accumulation of cyclic guanosine monophosphate (cGMP). High concentrations of cGMP are responsible for the atony of the vascular muscles of the arteries in and around the cavernous body which allows an increased bloodstream and consequently also the erection. The erection ends when cGMP is catabolised. The enzyme phosphodiesterase-5 (PDE-5) causes the reduction of the cGMP level and therewith the decrease of the erection.
How does Cialis help with erectile dysfunction?
This is the point where Tadalafil intervenes. As a PDE-5-inhibitor it blocks the phosphodiesterase-5 and makes an extended high level of cGMP possible. This brings about an elongation and intensification of the erection because of the lasting relaxation of the vascular muscles. This mechanism illustrates that Tadalafil is only useful during sexual arousal and cannot cause it. Therefore this substance is not an aphrodisiac which causes an erection without stimulation.
As Cialis has a half-time of 17,5 hours its effects can last for more than 24 hours up to 36 hours.
This quality brought it the nickname “weekendpill” as only a single dose is needed.
The tablet should ideally be taken 30 minutes before the sexual act.
Although the administration of PDE-5-inhibitors at women with female sexual dysfunctions has been described in literature there is no assured indication for Cialis in this context.
Contraindications for the use of Cialis (tadalafil)
- simultaneous administration of other pharmaceuticals which increase the virility
- children and teenagers under 18 may not use Cialis
- high blood pressure
- in case of priapism (permanent erections) or deviation of the penis (anatomic distortion) Tadalafil should be used only after the consultation of a doctor. Permanent erections have to be treated immediately, otherwise the tissue of the penis can be damaged severely or the virility can even be limited permanently
Side effects of Cialis (tadalafil)
This listing of the most frequent side effects contains unwanted effects which can occur but need not to because everyone reacts differently to certain pharmaceuticals.
In suspected cases of an allergic reaction Cialis must be recessed immediately and one should turn to a doctor.
While taking Cialis the following could occur: headache, dizziness, muscular pain and arthralgia, abdominal distension, gastric illness with an early feeling of satiety, pyrosis, epigastric pain, sickness, vomiting, constipation of the nose, flush, backache, conjunctivitis
Interaction of Cialis with other pharmaceuticals
A simultaneous administration of Cialis with other pharmaceuticals which contain nitrate or NO-donators (like the scene drug Poppers) is a contraindication.
The blood pressure could drop massively and cause a life-threatening hypotensive state. Some pharmaceuticals which effect the cardiovascular system also contain nitrate and should not be taken together with Cialis. Antibiotics and grapefruit juice can increase the side effects of Cialis.
